• Get a copy of your therapy records.
• Correct or amend your therapy records.
• Request confidential communication.
• Ask us to limit what we share.
• Get a list of those with whom we’ve shared your information.
• Choose someone to act on your behalf.
• File a complaint if you believe your privacy rights have been violated.
How We Use & Share Your Information
We may use and share your PHI in the following ways:
1. For Treatment- We may use your health information to provide, coordinate, or manage your treatment and related services.
2. For Payment- We may use and disclose PHI to process payments for services rendered.
3. For Healthcare Operations- We may use and share your PHI to improve our services, train staff, or conduct quality assessments.
4. As Required by Law- We will share information if required by state or federal law, including in the following circumstances:
• Reporting child, elder, or dependent adult abuse or neglect.
• If you are a danger to yourself or others.
• If a court orders the release of your records

Confidentiality & Limits of Privacy

3. If we are court-ordered to release records or testify.
